We believe that serving is normal in the body of Christ and getting involved is easy. Please follow the simple steps below:

Fill out the personal information section of the form.
Please look over the list of ministry opportunities provided and pray about where God would use you.
Check the box(es) indicating in which ministry you would like to serve.
Return this form to the church office.
The office will put you in touch with the leader of the ministry you are interested in, so that you can get started serving. You will also be invited to the next Servant's Quarters.
What is Servant's Quarters?
Servant's Quarters is an orientation meeting with Pastor Jack. He will share his vision for ministry as well as other important information about Calvary Chapel Chino Hills. It is our desire that everyone attend Servant's Quarters before getting involved in a ministry. If you are unable to attend this meeting, please contact the church office. Additionally, you can check your bulletin for the date of the next Servant's Quarters meeting.
